Lincoln Heritage Public Library Teacher Materials Request Form

The Lincoln Heritage Public Library is dedicated to partnering with all schools in local school districts to ensure children’s success. We gather and hold materials that support public, private, and parochial teachers’ needs.  The Teacher Materials Request Form is designated to make this process more efficient.

We ask that you allow at least two (2) days before the items are needed. This allows our staff to manage many requests at once and to select the best materials possible to fill these requests. You may bring the Teacher Materials Request Form to the library, fax your request to (Attn: Mrs. Shannon) @ 812.937.7102, or complete the form below and email it to shannon@lincolnheritage.lib.in.us (cc to lincolnheritage17@gmail.com). Please call after faxing to ensure that we received your request 812.937.7170.

Please provide us with as much information as possible on your subject (i.e., “space travel,” “friendship,” etc.) so that we can select materials most suitable for your curriculum and age group.  Specific titles may be requested, but are subject to availability at the time you make your request.  You can put specific titles on hold through the Evergreen System, but they will be subject to availability at the time of the pull.

You can check out up to 150 items on your Teacher Card due in 12 weeks.

You must bring your library card to check out items, pay library fines, place items on hold or access your library account.  If your library card is lost or stolen, report it to the library immediately.  You are financially responsible for any materials, fines, or fees attached to the account before the card is reported lost or stolen.

The online catalog at https://evergreen.lib.in.us/eg/opac/home is the most efficient way to request specific titles.  Don’t hesitate to contact any front desk library staff member for assistance with the catalog or this form.
